We do participate in other insurance plans - please call our office if you do not see your insurance plan listed.
PLEASE NOTE: Please bring your insurance card to all office visits in order for us to verify your benefits.
We reserve the right to charge $30 for patient missed routine appointments and $50 for missed physical exams. You will not be charged if you cancel at least 24 hours prior to your scheduled appointment.
Payment for services, including patient co-payments and patient balances, is expected at the time of service. We reserve the right to charge a $10 handling fee if your co-payment is not made at the time of service.
Please download and complete the patient registration and confidential patient history forms prior to your initial visit to our office. Please notify the receptionist if there are any changes to your insurance information, your address or phone number, or your name .
